Is closure report of a previous passport application is mandatory??

Hi,

I have applied for a passport from bhopal one and a half years ago.after some days passport status is shown as "please submit PPF forms" and later i moved out of bhopal and could not tarck it.
Now i see the status as " "Application has been closed, Please reapply quoting earlier File Number or contact PRO".

Now, i stay in bangalore and i want apply for a passport.So, is a closure report for the previous application is mandatory before applying for a new one? or can i apply for a new one without having the closure report?
Please help me in this issue how to proceed.
