Since your question is still not replied, hopefully it will also help other in future.
Because you have missed all your 3 appointments allotted. Now you can do following things:
1. Visit your APO (Assistance Passport Officer) with your application number and explain reason for missing the appointments. If he is convinced then he may allow you to submit documents or give you date. However, if its more than 6 months then consider your application already rejected.
2. If he denies or its been more than 6 months then apply for new passport application but make sure you select "YES" where you are asked to "If you have applied for passport in past".
