My mobile is asking PUK code, from where can i get it

I went to office without my mobile and came back to home, when i opened my mobile for a call it is asking me to provide PUK code and enter new pin, now where can i get this code

I think someone was trying to unlock your phone , may be when you were not near your phone in office. Now you should call company customer care with other sim and ask them to give PUK number, they will ask your details to verify and then will give it to you.
