This is called personalized chequebook and it is easily available if you ask bank to deliver to your home. You can apply online also through your netbanking (if you have enabled it). You have not mentioned the bank you are using , but almost all banks allow you to have option to apply for new chequebook online. Also, request made for checkbook online always gives you personalized check books ie. name on it.
You can also visit your bank and submit request in written to send you cheuque book at home, then also you will get name printed on it.
