I have lost my closure report, given by the PSK. Now, I want to apply for a passport afresh. Will I require the closure report? Since I have lost it, how can I get another one?

Do you have any reference number of file number of closure report ?
Thing is that you if your old passport application was closed then you an apply for new one with confidence given that you have proper explanation that why you got previous application closed. And you carry all proof with you.
However, if you are hiding any information from previous application then could be issue and you may even face penalty charges. In case you have some mail / closure report number then you may first ask them for duplicate and then apply afresh.
